This training on the Fatal Five for Case Managers includes an overview of information from the Fatal Five Fundamentals courses with added direction on how to use this information as a Case Manager. The training will provide strategies they as non-clinicians can use to improve the health and safety of those they serve.

An HCBS Final Rule requirement modification refers to a change or exception to the standard requirements of the Home and Community-Based Services (HCBS) Final Rule, which mandates that Medicaid beneficiaries receive services in settings that are integrated into the community. These modifications are permissible only when justified by a specific, individualized assessed need and documented in the person-centered service plan.
